Models, please use this form to submit your information and photos for the Zodiac Model of the Month Contest.  Not all models will be selected as the chosen 5 to compete for the month so we appreciate your time and submissions.  We hold the right to use your photo and information for future contests.  You will be notified if or when chosen. Be sure to Read The Rules before submitting your info.
